It's time to update your student-athlete's registration in Final Forms. **RETURNING USERS** should log in and update their information, while **NEW USERS** must create an account and register their student. All parents or guardians of Vincennes Lincoln athletes are required to complete this process each school year. Paper copies of signature forms are not accepted. When filling out the forms, please select all sports your student may be interested in, even if they are unsure. It’s easier to remove a student from a sport later than to add one at the last minute. Student-athletes will not be allowed to participate in a team’s activities (including practices/summer activities) unless they are registered for that sport in Final Forms and have a valid physical on file. Coaches can only access athletes’ information once they’ve been officially added to the team by a parent/guardian or the athletic office. In addition to completing all required sections in Final Forms (for both the parent and the student), a valid 2025-26 Physical Exam Form must be submitted. The physical form must be dated April 1, 2025, or later and should be uploaded to the student-athletes' Final Forms account (Take a picture of the form with a phone and upload it to Final Forms. The form that the medical provider signs is the only document that needs to be uploaded). NOTE: Physicals used for eligibility during the 2024-25 school year will expire at the end of the school year, with the exception being those students still participating in spring sports. Athletes will not be permitted to participate in any team activities—including summer workouts, practices, weight training, conditioning, or competitions—until all required Final Forms e-signatures are complete and a valid physical is on file. The student and parent/guardian will need separate email addresses for registration. STEP ONE: (Parent/Guardian starts the process) 1. Visit the Final Forms site to register as a new user or log in if you’re a returning user and select the blue box above your student’s name to begin updating the 2025-26 school year forms: https://vincennes-in.finalforms.com/ *** BE SURE TO SELECT THE BLUE 2025-26 STUDENT REGISTRATION BUTTON ABOVE YOUR STUDENT’S NAME 2. NEW ATHLETE REGISTRATION: Create an account and complete all steps. 3. RETURNING ATHLETE UPDATE: Verify each form in Final Forms. 4. It’s preferred to upload the Physical Examination Form directly to Final Forms. Alternatively, you can submit a paper copy to the LHS Athletic Office. The only document required to be uploaded or turned in to the athletic office is the Physical Examination page containing the medical professional’s signature and the exam date. Physical Examinations MUST have been administered on or after April 1, 2025. 5. Physicals conducted during the school’s Physical Day are already documented in Final Forms and filed in the Athletic Office. 6. Complete all required e-signatures via Final Forms at https://vincennes-in.finalforms.com/. This step is completed AFTER the parent/guardian completes their e-signatures. TRANSFER STUDENTS: If you have attended another HIGH SCHOOL before your enrollment at Vincennes Lincoln HS, you must complete an IHSAA Transfer Form with the Vincennes Lincoln Athletic Office before participating in any athletic practices, summer activities, or contests.
